Contact requests
If you submit a contact request on a provider profile, we collect the details you enter: your name, email or phone number, preferred contact method and time window, an optional message, and your consent to be contacted. Contact intake is enabled only when Treomark has a trusted connection to the selected provider. No request is accepted when that connection is disabled.
What you should not submit
Please do not enter medical, health, or other sensitive information into any form on this site. The contact form exists only to request a callback from a provider. Treomark is not a healthcare provider, and this site is not a HIPAA-covered service.
Provider information
Provider profiles are built from provider-supplied details, public records, and independently reviewed sources. This is published business information about a practice, not personal information about you.
Technical information
Our hosting infrastructure may process standard request data, such as IP address and browser type, to serve and secure the site. The public site does not set advertising or third-party tracking cookies.
How we use information
- To pass a contact request to the specific provider you chose to contact.
- To operate, secure, and improve the directory.
- To respond to questions a provider or visitor sends us.
How we share information
We share a contact request only with the provider it is addressed to. We may rely on infrastructure vendors (such as hosting and security providers) to operate the site, under appropriate confidentiality, and we may disclose information where required by law. We do not sell your personal information.
Cookies
Treomark's public site uses only what is necessary to function. It does not set advertising or analytics tracking cookies, and it does not build advertising profiles about you.
Retention and security
We keep submitted data to a minimum. No contact request is stored unless trusted intake is enabled. Where requests are accepted, we retain them only as long as needed to act on them, then remove them.
Your choices
You can ask us to access, correct, or delete a contact request you submitted. Providers can claim or correct a profile at any time.
Children
Treomark is intended for adults and is not directed to anyone under 18. Please do not use the site or submit information if you are under 18.
