Hair restoration compared: PRP, exosomes, minoxidil, finasteride, and light
PRP, exosomes, minoxidil, finasteride, and low-level light therapy differ in evidence, approval, route, maintenance, and the hair-loss types they are meant to address. Diagnosis comes first.
PRP and exosomes are not equivalent hair injections, and neither replaces a diagnosis. Minoxidil, finasteride, low-level light therapy, PRP, and exosome products differ in regulatory status, evidence, route, cost, and maintenance. The right comparison begins with the type of hair loss and whether follicles remain capable of producing hair.
Pattern hair loss, telogen effluvium, alopecia areata, scarring alopecia, traction, and scalp disease do not share one treatment plan. A camera image or package should follow the examination, not substitute for it.1

Minoxidil and finasteride are medication decisions
Minoxidil’s route matters. Topical formulations require regular application and can irritate the scalp; oral minoxidil is a separate prescription decision with systemic monitoring considerations. Ask whether the plan uses an approved product or a compounded combination and what happens when treatment stops.
Finasteride questions include who is eligible, the prescribed indication, side effects, and reproductive precautions. FDA’s 2025 alert says there is no FDA-approved topical finasteride formulation and describes adverse-event reports involving compounded topical products.5 A topical route does not automatically mean only local exposure.
What PRP evidence says
PRP is autologous: blood is drawn, processed to concentrate platelets, then injected. A 2023 meta-analysis of nine randomized trials involving 238 patients with androgenetic alopecia found improved hair density at three and six months compared with placebo, while some other measures did not differ significantly.2
Protocols remain variable. Ask about the blood volume, collection system, spin method, platelet concentration or characterization, activation, injection depth and spacing, number of sessions, maintenance, and how progress is photographed or measured. “PRP” alone does not tell you the dose.
Exosomes are a product, not your own plasma
An exosome product comes from a manufacturing source and is biologically different from PRP. FDA states there are no FDA-approved exosome products.3 That makes exact product identity, source cells, characterization, sterility, storage, route, and product-matched evidence necessary questions.
Do not use PRP evidence to fill an exosome evidence gap. A side-by-side clinic package or a paper discussing both categories does not make their sourcing, regulatory status, or trials equivalent.
Low-level light is a protocol
Photobiomodulation studies in androgenetic alopecia have found improved hair density compared with control.4 The relevant intervention is not just “red light”: it is a named device or parameter set, wavelength, irradiance, dose, schedule, and duration. A cleared cap or comb should be checked by exact model and indication; an in-clinic dome and an at-home helmet may not reproduce the same exposure.
Combination plans need a reason for each part
Clinics often combine medication, PRP, supplements, light, or procedures. Ask what each component adds, whether the combination has been studied, which element begins first, and how you will know which caused benefit or a reaction. Starting five interventions on one day may make attribution difficult.
Also ask what is maintenance-dependent. Medication and light usually require continued use to sustain an effect. PRP programs commonly include an initial series and later sessions. Exosome schedules vary because products and protocols are not standardized.
A consultation sequence
- Establish the diagnosis. Document the pattern, tempo, scalp findings, relevant history, medicines, and selected laboratory questions.
- Choose a target. Define reduced shedding, stabilization, density, diameter, or scalp symptom improvement.
- Compare evidence by diagnosis. Use studies in the same hair-loss type and avoid borrowing results from another condition.
- Identify the exact product or protocol. Name the drug, compounded formula, PRP method, light device, or exosome product.
- Review route-specific risks. Separate topical, oral, injected, and light exposures and identify the clinician responsible.
- Price the full course. Include monitoring, supplies, initial series, maintenance, and likely duration.
- Measure before changing course. Review standardized evidence at the agreed interval rather than judging from day-to-day shedding.
A practical decision framework
Supplements deserve a separate line in the plan. A branded hair vitamin can be convenient, but its presence in a package does not establish a deficiency or add evidence to PRP, light, or medication. Ask which ingredient addresses which finding, whether laboratory testing supports it, and whether doses overlap with other products. More components can raise cost and make benefit or side effects harder to attribute.
Ask how progression is handled even when treatment works. Pattern hair loss may continue in untreated follicles, so a good result in one area does not eliminate long-term planning. Clarify whether the goal is stabilization, regrowth, improved caliber, or camouflage and whether a transplant consultation is a present alternative or a later option. Procedures that preserve existing hair and surgery that redistributes follicles answer different questions.
Start with diagnosis, then rank options by evidence for that diagnosis, product clarity, burden, reversibility, monitoring, and maintenance. PRP has a growing but variable evidence base; exosome products carry a different and less settled product question; medications and light have their own route-specific tradeoffs.
