Spider veins vs varicose veins: when duplex ultrasound changes the workup

Spider veins are small intradermal vessels; varicose veins are larger dilated subcutaneous veins. Appearance alone does not show deeper reflux. Guidelines do not support routine duplex for every asymptomatic cosmetic telangiectasia, while symptoms, recurrence, bleeding, or skin change can alter evaluation.

Spider veins and varicose veins are not the same vessel class. Spider veins, or telangiectasias, are small intradermal vessels; varicose veins are larger, dilated subcutaneous veins. The surface pattern alone does not establish deeper venous reflux. Guidelines advise against routine duplex ultrasound for every asymptomatic cosmetic C1 telangiectasia, but symptoms, recurrence, bleeding, skin change, or suspected deeper disease can change the workup.1

The important sequence is classification, symptom history and examination first, then a reasoned decision about imaging. This article cannot diagnose a vein condition or determine whether an ultrasound or treatment is appropriate for an individual.

Classify the presentation before choosing a cosmetic modality

The CEAP clinical classification used in venous guidelines separates telangiectasias and reticular veins (C1) from varicose veins (C2) and from edema, skin change, and ulcer categories.1 A consumer does not need to self-assign a stage; the framework shows why every visible blue or red line should not be sold the same service.

Document pregnancy history, clot or thrombophlebitis history, prior vein procedures, ulcers, wounds, mobility, family history, compression use, and symptoms that vary with standing. The clinician should explain which facts change the pathway.

What duplex ultrasound adds

Duplex combines anatomic imaging with flow assessment. In a venous evaluation, it can map named veins, reflux, obstruction, connections, and response to maneuvers under a defined protocol. The report should identify the laboratory or practice, date, patient position, vessels examined, measurements, and interpreting clinician.12

It is not a generic “circulation test,” and a normal or abnormal line in a report does not choose a cosmetic procedure by itself. Ask what question the study is intended to answer and which decision depends on it.

Current guidelines state that routine duplex evaluation should not be performed in asymptomatic patients with telangiectasias or reticular veins because it may lead to unnecessary treatment of superficial truncal veins. In symptomatic C1 patients, evaluation may be considered to exclude associated venous incompetence; bleeding and more severe symptoms strengthen the rationale for medical evaluation.1 This is a bounded recommendation, not a ban on imaging or a requirement that every symptom leads to a scan.

Feeder veins and deeper reflux are different questions

A clinician may use transillumination, visualization, ultrasound, or another method to identify a local reticular feeder for a surface cluster. That local mapping should not be confused with a complete reflux study of the deep and superficial venous systems.

Ask the practice to name which map it is performing, who interprets it, and whether treatment is directed at a visible vessel, local feeder, refluxing truncal vein, perforator, or another target. This protects against treating a surface pattern repeatedly without explaining recurrence—and against escalating every cosmetic cluster into unnecessary truncal intervention.

Urgent symptoms do not belong in a cosmetic queue

Sudden one-sided swelling, pain, redness or warmth; chest pain; shortness of breath; uncontrolled bleeding; a new wound; or other acute concerning symptoms require timely medical assessment rather than a routine aesthetic booking. The clinic’s intake should say how it routes these concerns.

Choose sclerotherapy or laser only after the workup

Once vessel identity and any deeper issue are addressed, compare treatment by diameter, depth, color, location, skin context, feeder anatomy, product or device, endpoint, sessions, compression, and adverse-event plan. The separate sclerotherapy versus laser guide owns that modality decision.

  1. Record the pattern. Map fine surface vessels, larger bulging veins, edema, skin changes, wounds, and treated areas separately.
  2. Add symptoms and history. Document laterality, timing, standing relationship, bleeding, clot history, pregnancy, prior procedures, and recurrence.
  3. Define the imaging question. Ask what a duplex would measure, what protocol applies, and which decision its result could change.
  4. Assign medical ownership. Identify who interprets deeper disease, routes acute concerns, and coordinates care beyond surface appearance.
  5. Then compare modalities. Price the same vessel map, treatment target, number of sessions, follow-up, compression, and complication plan.

The decisive consultation question is: “What exact vessel class and source are you treating, what symptoms or findings support—or do not support—duplex imaging, and who owns any deeper venous result?”
