Wegovy for MASH: which fatty-liver diagnosis fits the approval?
Wegovy injection has accelerated FDA approval for adults with noncirrhotic MASH and F2–F3 fibrosis. Fatty liver alone does not establish that population. Trial endpoints used biopsy, while clinical assessment may use noninvasive tests; clinical benefit remains under confirmation.
Wegovy injection, not every semaglutide product or Wegovy tablet, has accelerated FDA approval for treating adults with noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) and moderate to advanced liver fibrosis consistent with F2–F3. The trial used biopsy-defined endpoints, but a biopsy is not automatically required to evaluate every patient. Continued approval may depend on a confirmatory clinical-benefit trial. “Fatty liver” alone does not establish the labeled MASH-and-fibrosis population.123
The decisive question is diagnostic specificity. Fat in the liver, MASH, fibrosis stage and cirrhosis are different findings. The current label does not turn a broad wellness or weight-loss advertisement into a liver-disease treatment claim for everyone with an abnormal test.
Build the two-axis record: disease and fibrosis
| Record | What it answers | What it does not establish alone |
|---|---|---|
| Steatosis or a fatty-liver note | Fat is present or suspected | MASH with F2–F3 fibrosis |
| MASH assessment | Inflammatory liver-disease context | The exact fibrosis stage or absence of cirrhosis |
| Fibrosis assessment | Extent of scarring or probability of a stage | That the treatment population matches the trial without clinical interpretation |
| Wegovy product label | Which formulation and population FDA reviewed | That another semaglutide brand inherits the MASH indication |
The ESSENCE trial enrolled people with biopsy-defined MASH and F2 or F3 fibrosis, and its planned interim analysis evaluated liver histology at week 72.2 The AASLD update explains that many candidates can be identified through noninvasive tests and that biopsy is impractical or unnecessary for most; trial inclusion criteria are not a universal clinical testing order.3 A reader should not self-assign a fibrosis stage from one number. Ask the liver clinician which evidence supports the diagnosis and how it compares with the labeled population.
Understand what accelerated approval says
Accelerated approval is a real FDA approval with a defined indication, not an experimental-product status. It can use an endpoint reasonably likely to predict clinical benefit while a confirmatory trial continues. Here the label specifies improvement in MASH and fibrosis, and says continued approval may be contingent on verification and description of clinical benefit.1
The phase 3 interim results showed more histologic responses with semaglutide than placebo in the studied population.2 They do not yet settle how much the treatment changes long-term outcomes such as decompensated cirrhosis, transplantation or liver-related death. A sales claim that skips from biopsy improvement to a guaranteed clinical outcome exceeds the cited evidence.
Keep brand and route attached to the claim
Semaglutide appears in several named products. The MASH indication in the current Wegovy label belongs to the injection; Wegovy tablets have different stated indications, and Ozempic is a separate product.1 The four-brand indication guide maps those names. A clinic should write the exact proposed product, formulation, purpose and FDA status rather than market “semaglutide for fatty liver” as if the ingredient alone were the approval.
Weight-management care and liver-disease care can overlap, but they are not the same consultation. Ask who established the MASH diagnosis, who follows fibrosis and liver outcomes, how other liver conditions were considered, and how the weight-care prescriber and liver clinician will share records. The label also contains contraindications and safety warnings that require an individual clinical review; this article cannot decide eligibility.
At a medical weight-management consultation, bring the liver reports and the exact product proposal. The strongest answer names both the disease stage and the formulation, then states honestly which outcomes are already supported and which remain under confirmation.
