CLIA-certified lab vs FDA-authorized test: two oversight layers, two different claims
CLIA certification addresses the laboratory and its testing quality systems. FDA approval, clearance, or authorization addresses a specific test and intended use. A lab certificate does not convert an LDT into an FDA-authorized test, and an authorized kit still must be performed under the applicable laboratory rules.
A CLIA certificate and FDA authorization answer different questions. CLIA regulates the laboratory site and the quality of testing on human specimens. FDA approval, clearance, or emergency authorization applies to a specific test, intended use, specimen, user, and labeling. A CLIA-certified lab does not make every test it offers FDA authorized; an FDA-authorized test still must be performed in the laboratory setting and complexity category its labeling requires.12
The safest way to evaluate a wellness or screening test is to verify two records side by side: the laboratory’s current CLIA certificate and the exact test’s FDA or laboratory-developed-test status. Then ask a third question that neither record alone answers—will the result improve a real decision?
Keep the oversight layers separate

“Certified test” is therefore incomplete. Ask: certified by whom, at which level, for the laboratory or the assay, and for which claim?
CLIA follows the testing site
CLIA applies broadly to U.S. facilities or sites that test human specimens for health assessment or to diagnose, prevent, or treat disease, subject to defined exceptions.1 CMS administers certificates and inspections, CDC provides scientific and technical support, and FDA categorizes tests by complexity.
A specimen-collection office may not be the testing laboratory. The tube can be drawn in South Florida and shipped to another state. Record both the collection site and the legal laboratory named on the report, including CLIA number, address, laboratory director, and test method.
Certificates match the work performed. A Certificate of Waiver permits only waived testing. Nonwaived moderate- and high-complexity testing carries additional quality-system, personnel, proficiency-testing, and inspection requirements.2 A certificate number without certificate type and current status is only half a verification.
Waived does not mean clinically trivial or foolproof
FDA may categorize a test as waived because it is simple and has a low risk of erroneous result when used according to instructions. CDC emphasizes that waived tests can still produce serious errors when instructions are not followed or users are unfamiliar with the system.2 The site still needs an appropriate CLIA certificate and must follow the manufacturer’s directions.
Changing specimen type, timing, storage, reader, cutoff, software, or intended population can move the real-world use away from the reviewed instructions. Ask whether the practice performs the test exactly as labeled. If it modifies an authorized assay, request the laboratory’s validation and the result report’s disclosure of the modification.
Home collection introduces another chain: kit authorization, collection instructions, stability, shipping conditions, accession, rejection criteria, and identity matching. A CLIA logo on a sales page does not show that your specimen stayed within those conditions.
FDA status belongs to the exact test and claim
FDA uses different pathways for tests. Premarket approval, 510(k) clearance, De Novo classification, and emergency use authorization are not interchangeable. The database record and labeling identify what specimen, condition, population, setting, user, and result the agency reviewed.
A genetic health-risk report, carrier screen, pharmacogenetic association, diagnostic assay, cancer screen, and general-wellness interpretation can use similar laboratory technology while making different claims. Do not transfer authorization from one report module to another.
FDA’s direct-to-consumer test guidance also distinguishes access from interpretation: some authorized tests can be purchased directly, while others require a prescription or professional involvement, and results may need confirmation or clinical context.4 “Available online” is not a regulatory category.
LDT status changed in 2025—use the current page
In May 2024, FDA issued a final rule addressing in vitro diagnostics manufactured by laboratories. A federal district court vacated that rule on March 31, 2025. On September 19, 2025, FDA reverted the regulatory text to its pre-rule wording.3 Older explainers that present the 2024 phaseout schedule as current can now misstate the operative landscape.
An LDT offered in 2026 should not be described as FDA approved or cleared unless an exact FDA authorization record supports that test. Nor should “FDA does not regulate this test” be used as a blanket quality claim. Ask the laboratory to state plainly:
Analytical validity is not clinical utility
Analytical validity asks whether the assay accurately and reliably measures the target. Clinical validity asks how well that measurement relates to a condition or outcome in the intended population. Clinical utility asks whether using the result improves a meaningful decision or outcome compared with alternatives.
CLIA quality systems support reliable laboratory performance, but a CLIA certificate does not prove that a biological-age score, food panel, multi-cancer signal, or medication recommendation has demonstrated clinical utility. FDA authorization for a narrow claim also does not authorize a seller’s broader wellness interpretation.
Ask for absolute counts, not only accuracy percentages: how many people were tested, how many had the condition, how many positive and negative results were correct, how many were indeterminate, and whether validation resembled the people being offered the test. Prevalence changes positive predictive value, so a high sensitivity or specificity headline may not predict what a positive means in a low-risk wellness population.
Build a result-to-action contract before ordering
Write what happens for positive, negative, indeterminate, and discordant results. Identify who explains the result, what confirmatory method is used, whether ordinary screening continues, what follow-up can cost, and what happens if the reported risk has no validated intervention.
For a test sold through a med spa, ask whether the treating professional is ordering it, whether a separate telehealth clinician signs the order, and which laboratory issues the report. A dashboard summary may be generated by a different company than the underlying lab result.
Verify the two records and the third question
- Identify the testing laboratory. Use the report or requisition—not the storefront—to capture legal name, location, CLIA number, certificate, and laboratory director.
- Identify the exact assay. Record test name, version, method, specimen, collection kit, intended use, and result modules.
- Check FDA status literally. Find the approval, clearance, De Novo, or authorization record—or document that it is offered as an LDT without FDA premarket review.
- Match the use to the record. Compare population, specimen, user, condition, cutoff, and claim with the proposed use and any modifications.
- Read the validation denominator. Separate analytical performance, clinical validity, predictive value in the offered population, and clinical utility.
- Pre-plan follow-up. Assign interpretation, confirmation, ordinary screening, cost, records, and the response to an uncertain result.
The decisive question is: “What does this laboratory’s CLIA certificate establish, what exact FDA status does this test have, and what evidence shows that acting on this result helps someone like me?”
