GLP-1 drugs and prediabetes: trial results versus an FDA indication

Trials in adults with obesity and prediabetes found improvements in glycemic categories and diabetes incidence with specific incretin medicines. Prediabetes alone is not a stand-alone FDA indication for Wegovy or Zepbound; eligibility depends on the exact product's labeled population and the full clinical picture.

Specific incretin medicines improved prespecified outcomes in trials that enrolled people with both obesity and prediabetes. STEP 10 tested semaglutide and measured weight and return to normoglycemia; a SURMOUNT-1 extension tested tirzepatide and measured progression to type 2 diabetes. These results do not create a stand-alone FDA indication to treat prediabetes. Wegovy and Zepbound must be assessed against each exact product’s current labeled uses, population criteria and safety information.1234

Prediabetes is a useful clinical finding, but it does not answer every question a medicine label or trial asks. The reader needs to separate a study population, a measured result and a product indication. A clinic that collapses those three into “approved for prediabetes” has changed the claim.

Match the trial to the question

STEP 10 randomized 207 adults with obesity and prediabetes to semaglutide 2.4 mg or placebo alongside lifestyle intervention for 52 weeks. Its dual primary outcomes were weight change and the proportion with normoglycemia, followed by an off-treatment observation period.1 A return to a normal glycemic category at one study point is meaningful, but it is not a permanent cure claim. Trial eligibility also prevents us from applying its percentage to every person with a mildly elevated laboratory result.

The SURMOUNT-1 diabetes-prevention analysis followed a prediabetes subgroup with obesity for 176 weeks of tirzepatide or placebo, then observed a shorter period after treatment ended. It measured onset of type 2 diabetes in that defined cohort.2 That endpoint and follow-up differ from STEP 10. The studies cannot be combined into a simple “GLP-1s prevent diabetes by X%” class-wide number; tirzepatide is itself a dual GIP/GLP-1 receptor agonist.

QuestionWhat the evidence can answerWhat it cannot answer alone
Did a trial include prediabetes? The enrolled population and its inclusion criteriaWhether every person with prediabetes is a labeled candidate
Did glucose status improve?A prespecified endpoint at a stated time pointWhether the effect persists after treatment for everyone
Was diabetes onset lower?Incidence within the studied cohort and follow-upAn FDA diabetes-prevention indication for the product
Is the medicine approved for this visit's goal? The exact brand, formulation and current labelA clinical decision without the person's full history

Read the label before the headline

Wegovy and Zepbound have chronic weight-management indications for defined populations; their labels contain other specific uses and warnings too.34 A person with prediabetes may also meet a weight-management indication, but the qualifying basis would be the labeled population, not a newly invented prediabetes indication. The four-brand indication guide helps keep semaglutide and tirzepatide products attached to their own approvals.

Likewise, a trial result can inform a clinician’s discussion without expanding the label. An advertisement saying “diabetes prevention treatment” should name the exact study, whether the target is weight or glycemic status, and the FDA status of the proposed product. It should not substitute a manufacturer ingredient name for a product-specific approval.

Bring an interpretable record to the visit

Ask which laboratory result established prediabetes, when it was measured, and whether it has been repeated or interpreted alongside other conditions. Bring current medicines, weight-related diagnoses, prior care attempts and any relevant kidney, gastrointestinal or other history. The prescriber can then explain which product’s criteria are being considered and what outcome the plan will actually follow.

A useful medical weight-management consultation should distinguish three statements in writing: “this trial found an effect,” “this product is labeled for this use,” and “this person is an appropriate candidate.” They are related, but none replaces the others.
