Alzheimer's blood tests: FDA clearance is not a stand-alone wellness screen
FDA clearance of a named Alzheimer's blood test is product- and intended-use-specific. The first cleared plasma test is an aid to evaluate amyloid pathology in adults 50 and older with symptoms in specialized care; it is not intended for general screening, stand-alone diagnosis, prognosis, or treatment selection.
A blood test marketed for Alzheimer’s disease should be matched to its exact FDA record and intended use. The indications for use in FDA’s decision summary for the first cleared plasma test—the Lumipulse G pTau217/β-Amyloid 1-42 Plasma Ratio—cover adults 50 and older who have signs or symptoms of cognitive decline and are being evaluated in specialized care.3 The test is not intended as a screening or stand-alone diagnostic test, and results must be interpreted with other clinical information.13
That bounded clearance should not be expanded to every pTau, amyloid, “brain age,” dementia-risk, or wellness panel.

Read the clearance as a sentence with every qualifier
FDA cleared K242706 through the 510(k) pathway in May 2025.12 The decision concerns one named assay that measures pTau217 and beta-amyloid 1-42 in plasma and calculates a ratio. The intended-use language identifies age, signs and symptoms, specialized-care context, comparison to amyloid pathology, and the need to combine results with other clinical information.3 FDA’s announcement summarizes the population as adults 55 and older, while the device decision summary states 50 and older; for the exact indication, use the product-specific decision record rather than silently reconciling the two descriptions.13
Preserve each qualifier:
- Product: the exact Lumipulse ratio assay, not every blood pTau217 test.
- Specimen and system: plasma tested on the specified laboratory platform, not a home strip or a different assay.
- Population: adults 50 and older with signs or symptoms of cognitive decline, not asymptomatic wellness customers.
- Job: an aid in assessing amyloid pathology, not a complete diagnosis of the cause of symptoms.
- Setting: specialized care, not an unattended consumer dashboard.
- Interpretation: together with other clinical information, with a pathway for positive, negative, and indeterminate results.
“FDA cleared technology” on a seller’s site is incomplete until the ordered test matches that record.
Clearance and approval are not interchangeable words
FDA reviewed the Lumipulse submission as a 510(k), finding substantial equivalence to a legally marketed predicate device using cerebrospinal fluid.1 It is accurate to call K242706 cleared, not FDA approved. Breakthrough Device designation facilitated interaction and review; it is not a separate effectiveness approval or a broader intended use.
A laboratory may also offer an assay that was not the subject of FDA premarket review. CLIA certification concerns laboratory quality requirements; it does not turn every assay on the menu into an FDA-cleared test. Ask the lab to state in the report whether the test is FDA cleared, approved, authorized under another pathway, or a laboratory-developed test.
Performance numbers travel only with the study conditions
FDA’s announcement describes the pivotal evaluation using 499 plasma samples from cognitively impaired adults and reports how positive and negative results corresponded with amyloid PET or cerebrospinal-fluid results, with an indeterminate category for a portion of participants.1 Those figures should not be copied to:
- a different commercial assay;
- an asymptomatic screening population;
- a younger population;
- a home-collected specimen without matching validation;
- a different cutoff or algorithm; or
- prediction of future dementia, progression, or treatment response.
Predictive values can change when prevalence and referral setting change. Ask for the exact test’s sensitivity, specificity, positive and negative predictive values in the intended population, indeterminate rate, precision, interferences, and study limitations. Then ask how those properties apply to the person’s actual reason for testing.
A result is not the end of the diagnostic workup
Amyloid pathology can be relevant to Alzheimer’s disease, but symptoms can have multiple contributors and an amyloid result is not a complete cognitive evaluation. FDA warns that false-positive and false-negative results can lead to distress, unnecessary care, incorrect diagnosis, or delayed evaluation.1 The report needs a named clinician who can integrate history, examination, medications, function, other laboratory information, imaging, and specialist evaluation as appropriate.
The Alzheimer’s Association guideline resource frames blood-based biomarkers within a diagnostic workup for people with suspected disease and distinguishes tests that may triage from those that may confirm amyloid pathology under defined performance thresholds.4 A professional guideline does not expand an individual product’s label; use both the current guideline and the test-specific record.
Cost includes the interpretation pathway
Price the order, collection, laboratory assay, clinician interpretation, specialist evaluation, confirmatory testing, repeat testing, imaging, counseling, and records transfer. Ask about insurance authorization and what happens after an indeterminate or discordant result.
A discounted blood draw can be expensive if the program has no responsible clinician or if confirmatory care must be rebuilt elsewhere. Conversely, a test ordered inside a complete evaluation may reduce reliance on more invasive or resource-intensive assessment for some patients, but that value belongs to the actual clinical pathway—not the marker alone.1
Verify before consenting
- Capture the exact test. Record product, manufacturer, laboratory, specimen, platform, analytes, algorithm, and report language.
- Open the FDA record. Match the submission number, intended use, population, setting, limitations, and current decision summary.
- Define the clinical question. State the symptoms or finding being evaluated and why this test may add information.
- Plan every result category. Name the clinician and next step for positive, negative, indeterminate, or discordant findings.
- Price and protect the whole pathway. Include interpretation, confirmation, specialist care, records access, privacy, and data-retention terms.
The decisive question is: “Does this exact test’s FDA-cleared intended use match the person, symptoms, specimen, setting, and proposed decision—and who owns what happens after every possible result?”
